Appointment requests

Sending an appointment request through this site is a request, not a confirmed appointment. Nothing is booked until someone from our team contacts you and confirms it. Please do not rely on a form submission for anything time-sensitive — call us instead.

Accuracy

We try to keep this site accurate and current, particularly the insurance lists, which we update as our contracts change. We cannot guarantee that every detail is correct at every moment. Insurance coverage in particular depends on your specific plan and should always be confirmed with us or with your insurer before your first visit.
