Targeted Case Manager Supervisor

Job Description

Scope:

Under the general direction of the Executive Director, the Case Manager Supervisor oversees, monitors, and evaluates the Case Management program and coordinates Case Management services. The Case Manager supervisor is also responsible for training and supervising Case Management staff.

Job Responsibilities:

The Case Manager Supervisor will serve as the primary staff in case management and in developing, implementing, supervising, reporting, and evaluating the program’s service interventions.
 
Supervision and consultation with all staff regarding all aspects of recipients’ case management interventions. Such involvement shall include, but not be limited to:
 
  • Provide team leadership.
  • Plan supervision schedules, provide consultations in reference to guidelines of the policies and procedures and requirements of the facility and maintain documentation of supervision.
  • Assist with service planning.
  • Assist in planning for provisional services to the recipient as deemed appropriate by the team.

To ensure on-going communications and efficient reporting, the Supervisor will:

  • Serve as the liaison between the facility and the referring agency.
  • Represent the agency at any court hearing involving the recipient and will make reports based on team recommendations.
  • Write quarterly staffing reports based on input from the Quality Improvement (QI) team.
  • Responsible for oversight of any on-going paperwork necessary to facilitate services while the recipient is in active status.

Qualifications:

  • A master’s degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field and three years of full time or equivalent professional experience serving the target population; or
  • A bachelor’s degree from an accredited university or college and five years of full time or equivalent case management experience serving the target population; and
  • Bring to the position a current Certified Behavioral Health Case Manager Supervisor (CBHCMS) credential from theFlorida Board Certification (FBC).

Reporting Relationship:

This person reports directly to the Executive Director.
