Targeted Case Manager
Job Description
Scope:
Provide comprehensive case management for adults, children or adolescents and their families. The main functions shall be coordinating, monitoring, planning, linking and advocating for all of the recipients service needs. The case manager will serve as the recipients advocate in attempting to provide and facilitate the best possible care and treatment available.
Job Responsibilities:
- Conduct an initial intake on the recipient.
- Schedule and perform regular home and field visits and face-to-face contacts.
- Conduct comprehensive assessments, evaluating recipient’s current service needs.
- Develop service plans in collaboration with recipient and with the implementation of specific goals and objectives based on the identified needs of the assessment.
- Serves as advocate and liaison between recipient and community agencies, programs and resources.
- Record every activity, service or intervention on a daily basis via Progress Notes.
- Maintain recipient’s record and comply with documentation standards.
- Prepare progress reports and court reports (if applicable).
- Attend court hearings related to the recipient (if applicable).
- Communicate with psychiatrist, primary care physician and other professionals working with the recipient maintaining a collaborative relationship.
- Attend supervision, trainings and staff meetings as scheduled.
- Comply with productivity guidelines and submit weekly billing reports on a timely manner.
- Share all concerns with Supervisor, Executive Director or Designee.
- Prepare a termination summary prior to the recipient’s discharge from the program.
- Have knowledge of available resources within the community.
Reporting Relationship:
This person reports directly to the Case Management Supervisor.
Qualifications:
- Must have a bachelor’s degree (B. A. or B.S.) from an accredited university or college with a major in counseling,social work, psychology, criminal justice, nursing, rehabilitation, special education, health education or in any relatedsocial services or behavioral health field;
- A minimum of two (2) year of direct experience working with people with mental health conditions and emotionaldisabilities;
- Must have the AHCA-approved TCM Certificate.
- Must be certified by the Florida Certification Board (FCB) as Certified Behavioral Health Case Manager (CBHCM)or must have the provisional certification (CBHCM-P)
Requirements:
- A valid Florida driver’s license, insurance, reliable transportation and be capable of unrestricted travel in theperformance of his/her duties.
- Basic computer skills, fluency in English and Spanish preferred.
- Strong organizational, time management and data management skills.
- Knowledge of community resources.
- Awareness of the social, economic and cultural factors that affect families in our community.
