What is the purpose of Form N-648?
This form is filled out and certified by certain medical professionals on behalf of applicants who are requesting an exception from the English and/or civics requirements for naturalization because of a disability and/or impairment.
Who is eligible to obtain an N-648?
An applicant is eligible for this exception if they are unable to learn and/or demonstrate knowledge of English and/ or U.S. history and civics because of a physical or developmental disability, or mental impairment (or a combination
of impairments). The disability and/or impairment must result from anatomical, physiological, or psychological abnormalities, which can be shown by medically acceptable clinical and laboratory diagnostic techniques. The disability and/or impairment must result in functioning so impaired that the applicant is unable to demonstrate the required knowledge.
N-648 Frequently Asked Questions
To set up the Initial Interview appointment with the Psychologist, you must come to our office to complete the Intake package and provide the following documents.
- Valid Picture State ID
- Health Insurance ID Card (If applicable)
- Medical Records from prior treatments to support your disability (Ex: Medical Records, Psychiatric Evaluations, Psychotherapy Notes, Assessments, etc.)
Health Insurance companies only pay for medically necessary services. Getting evaluated with the only purpose of obtaining a Waiver (N-648) for the USCIS exam is not considered medically necessary by some insurance companies. We recommend the clients call their insurance before contacting us to determine if they are willing to cover the service.
The entire process is $600.00. However, if the Clinician determines you don’t qualify to obtain the waiver based on your medical history, no additional payment would be required besides the $300.00 you paid to schedule the Initial Interview appointment.

No, you will only receive an N-648 completed and signed by our Psychologists if it is determined that you qualify to obtain the waiver.
Yes, Occasionally, USCIS asks for additional supporting documentation to approve the waiver. However, we only do revisions to clients who received the N-648 from our center. Clients evaluated at a different center would have to start the process from the beginning.
